What to Expect

Your transition to a skilled nursing facility can be a lot smoother if you have a realistic idea of what to expect. Here is short list of things that are helpful for you to know, especially if you are looking at skilled nursing for the first time. If you have additional questions, please do not hesitate to reach out for more information:

Our goal is for you to take your next step as quickly and as safely as possible. The length of everybody’s stay is different, and we want to work with your insurance company to make sure you receive the care you need to transition safely in the shortest time frame possible. Our therapy and nursing teams are here to provide you with a high level of support as you work towards your well-being. Within the first week, there also will be a comprehensive meeting regarding discharge plans and needs to ensure a smooth handoff.

The first couple nights’ sleep can sometimes be the toughest. A new environment can take some adjustment. If you need earplugs, an extra pillow, or something else to help make your rest more comfortable, please let the staff know.

Your initial visit will be within the first three days of your stay, and you may not see one again before you leave. Although this might raise some initial concern, it is an indicator that your health is getting better and you no longer need to see a doctor every day like you did in the hospital. Our nursing staff will be in consistent communication with your provider to address any questions and respond to any changes in your health conditions.

Medications you were taking at the hospital or at home may be different than the ones you will take at Mission Hills. A nurse will explain your medications once you arrive and work with you to make sure you have the appropriate prescriptions.

Our therapy team will be in to visit you within 24 hours of your arrival to assess your needs and develop a plan to help you reach your goals.

Your diet is designed for you. Our kitchen staff works to provide you with food that will aid in your healing. Our registered dietician and certified dietary manager will interview you to understand your preferences and find foods that will be most beneficial.

OUR EXPERT TEAM

Let our talented and focused staff help you regain your strength and mobility!

Visit Mission Hills Post Acute, and you’ll notice a definite difference: our caregivers are the best. You’ll meet a team of innovators—Nurses, Therapists and other Clinicians, all professionals at the top of their game, genuinely committed, remarkably caring and wholeheartedly compassionate.

More importantly, as leaders on the front lines of Daily Care, our Caregivers have a clear voice in the operation of their departments and the facility—and especially in the way they deliver care to their patients. Our Leadership Team treats our Caregivers the same way we want them to treat our Residents: with love, respect, and admiration for the wonderful people they are.

This feeling of Family and Caring permeates everything we do at Mission Hills Post Acute Care, and enables us to attract the most outstanding Caregivers our community has to offer. While in our care, please know that your loved one will be well taken care of. Because all of our rehab therapists and care providers are in-house (rather than outside contractors), our relationships with our residents are superior.

Come and see for yourself and let us exceed your expectations!

On-Site Decision Makers

We Don’t Wait To Make Important Decisions

Providing the best care possible dictates that Caregivers be empowered to make decisions—in many cases, every second counts. We leave the critical decisions to those who know best: the Caregivers, Doctors, and Nurses who work with our patients 24-hours a day, 7-days a week.

It’s important to us that our patients, as well as their family members, feel loved, informed and secure during their stay. Quality of life is just as important as quality of care in a skilled nursing facility! We work hard to keep patients happy and comfortable, and patients have consistently shown greater motivation in achieving their goals as a result. If there’s a special need or request, the Team Member has authority to act upon it.
